Satisfactory Academic Progress Appeal Form
Medical students who do not maintain the Satisfactory Academic Progress (SAP) as outlined by the WSU School of Medicine Promotions Committee will be placed on Financial Aid Probation and must submit the SAP Appeal and Personal Statement to be considered for continued eligibility.
Please check your requirements in WSU Pipeline and submit only the forms that are requested. If you submit incorrect or unnecessary documents, your award may be delayed.

*For students who are requested in Pipeline to submit the 2013-14 Verification Worksheet:
If you did not use the IRS Data Retrieval Tool when you filed your FAFSA or you used the Data Retrieval Tool and then made changes to your FAFSA, you must submit an official IRS Tax Return Transcript. NOTE: Federal financial aid regulations do not permit the SoM Office of Student Financial Aid to accept copies of federal tax returns.
The IRS Tax Return Transcript only includes information from the original tax return. If the tax return is amended, the IRS Tax Account Transcript will show the amendments but not all of the data that did not change. Therefore, if an amended return is filed, both an IRS Tax Return Transcript and an IRS Tax Account Transcript must be obtained from the IRS and submitted to OSFA - SoM for verification to be completed. To obtain an IRS Tax Return Transcript and an IRS Tax Account Transcript, go to www.IRS.gov and click on the “Order a Return or Account Transcript” link, or call 1-800-908-9946.
The IRS does provides another option for tax filers who filed an amended tax return – the Record of Account, which includes the required information from both the IRS Tax Return Transcript and the IRS Tax Account Transcript. However, the Record of Account can only be requested using a paper IRS Form 4506-T. A paper request, it may take up to 30 days to receive.
