Medical students who do not maintain the Satisfactory Academic Progress (SAP) as outlined by the WSU School of Medicine Promotions Committee will be placed on Financial Aid Probation and must submitan appeal form to be considered for continued eligibility.

*For students who are requested in Academica to submit the 2014-15 Verification Worksheet:

If you did not use the IRS Data Retrieval Tool when you filed your FAFSA or you used the Data Retrieval Tool and then made changes to your FAFSA, you must submit an official IRS Tax Return Transcript. NOTE: Federal financial aid regulations do not permit the SOM Office of Student Financial Aid to accept copies of federal tax returns.

The IRS Tax Return Transcript only includes information from the original tax return. If the tax return is amended, the IRS Tax Account Transcript will show the amendments but not all of the data that did not change. Therefore, if an amended return is filed, both an IRS Tax Return Transcript and an IRS Tax Account Transcript must be obtained from the IRS and submitted to OSFA - SOM for verification to be completed. Download and print the transcript from the IRS website: http://www.irs.gov/Individuals/Get-Transcript.

The IRS provide another option for tax filers who filed an amended tax return – the Record of Account, which includes the required information from both the IRS Tax Return Transcript and the IRS Tax Account Transcript.
